Pick Up PTO Frozen Food Sale Orders & Prizes
Wednesday, December 5, 2007   2:30 - 6:30pm
Intermediate School Cafeteria

PK thru 2nd grade - An ADULT* must pick up the order and sign for it.
(* = an older sibling may pick up the order with the completed permission
form below)
3rd thru 5th grade - Students may only pick up the order and sign for it
if they bring the       	completed permission form below.
*(Only cleared orders may be picked up by a student.   See below to see if
your order has 	been cleared)
It is very important that you pick up your order on this day because all
products are frozen.  The PTO does not have a place to store left over
items.  Please make arrangements to have your items picked up on Dec.
5th.All items not picked up on Dec. 5th will be donated to the Devine Food
Pantry.

needed for this PTO event.  Help is needed between 2:00-6:30pm on the
Intermediate School Cafeteria on 12/5/07.  Any help is appreciated.
